INVESTMENT: I do not bill insurance for ADHD evaluations as they are not typically paid for by insurance. They are a private pay service. Payment plans are available for those who

may need them.

Complete the Request an ADHD Evaluation Form

We will contact you to discuss all the details and get you started by sending intake paperwork. We will also ask you to permit us to request any relevant medical records. Once we have received your completed paperwork, we will schedule your initial interview.

You will schedule and attend your in-person interview.

These appointments typically last between 90-120 minutes. We review your paperwork and medical records and create a plan to determine which assessments and rating scales will be utilized to rule in or rule out a diagnosis of ADHD.

Complete and return all additional assessments and rating scales.

All of your information and data will be reviewed, and a comprehensive evaluation will be created.

Attend a final 60-minute review appointment.

During this appointment your results and treatment recommendations will be discussed with you. This is also an excellent time to ask any questions before your report is finalized.

Receive your final report.

One week after your follow-up appointment, you will receive your final report that clearly addresses your interview findings, assessment and rating scales, and feedback from your final interview. It will also outline your strengths and provide treatment recommendations.

Meet Melanie T. Twite, MA, LCPC

With over a decade of experience in diagnosing and treating ADHD, I understand the challenges patients face when seeking answers to treat their ADHD symptoms. Many individuals are referred to their primary care providers for ADHD-related concerns, only to leave with treatment plans focused on anxiety or depression. This often happens not because physicians are unwilling to help but because ADHD is a complex condition requiring specialized expertise.

That’s why I decided to offer ADHD evaluations as part of my services. By providing thorough, science-based assessments, I collaborate with doctors to ensure they have the information they need to make accurate diagnoses and recommend effective treatments. My goal is to help patients get the right care faster, empowering them to thrive.

As a Licensed Clinical Professional Counselor and ADHD-Certified Clinical Services Provider (ADHD-CCSP) I hold:

  • A Master’s Degree in Marriage and Family Therapy from George Fox University

  • Licenses in Idaho, Texas, and Wyoming

  • Advanced credentials in ADHD evaluations through the Institute of Certified ADHD Professionals.

  • Advanced training with Darlene Sousa, PhD., specific to ADHD evaluation.

  •  I am approved by Pearson Education as a Level C provider to administer, score, and interpret proprietary ADHD assessment tools.
